Basic Policy regarding the Proper Handling of Specific Personal Information, etc.

Our company hereby sets forth this Basic Policy in order to work as an organization to ensure the proper handling of specific personal information and individual numbers (hereinafter “Specific Personal Information, etc.”) based on the “Act on the Use of Numbers to Identify a Specific Individual in Administrative Procedures” (“My Number Act”). 

 

  1. Name of business operator

  2. Compliance with relevant laws, ordinances, and guidelines, etc.
    Our company shall properly handle Specific Personal Information, etc. and comply with the My Number Act, the “Act on the Protection of Personal Information,” and other laws and ordinances, as well as the “Guidelines for Proper Handling of Specific Personal Information (for Business Operators)” and other guidelines.
     
  3. Purpose of use
    Our company shall use Specific Personal Information, etc. for the following utilization purposes.
    (1)Specific personal information, etc. regarding executives and employees (including dependents) (including clerical tasks related to the following)
    (i) Clerical tasks, etc. related to withholding tax
    (ii) Clerical tasks, etc. regarding preparation of Application for (Change in) Exemption for Dependents of Employment Income Earner, and Application for Basic Exemption of Employment Income Earner and Application for Exemption for Spouse of Employment Income Earner and Application for Exemption of Amount of Income Adjustment
    (iii) Clerical tasks, etc. regarding preparation of salary payment report
    (iv) Clerical tasks, etc. regarding preparation of notification of transfer of salary earner related to salary-payment-report special tax collection
    (v) Clerical tasks, etc. regarding preparation of application for change to special tax collection
    (vi) Clerical tasks, etc. regarding preparation of payment records of recipients of retirement allowances, etc.
    (vii) Clerical tasks, etc. regarding preparation of return form for retirement income
    (viii) Clerical tasks, etc. regarding preparation of declaration, notification, and application forms concerning property-formation housing savings and property-formation pension savings
    (ix) Clerical tasks, etc. regarding notification of health insurance, nursing insurance, welfare pension, and corporate pension
    (x) Clerical tasks, etc. regarding notification of category III national pension
    (xi) Clerical tasks, etc. regarding applying for and claiming health insurance, nursing insurance, welfare pension, and corporate pension
    (xii) Clerical tasks, etc. regarding notification of employment insurance and workers’ accident compensation insurance
    (xiii) Clerical tasks, etc. regarding applying for and claiming employment insurance and workers’ accident compensation insurance
    (xiv) Clerical tasks, etc. regarding preparation and provision of legal documents concerning financial instrument transactions related to employee stock ownership association
    (xv) Clerical tasks, etc. regarding handling of group insurance 


    (2) Specific personal information, etc. regarding individuals who are not executives or employees (including clerical tasks related to the following)
    (i) Clerical tasks regarding preparation of payment report concerning remuneration and fees, etc.
    (ii) Clerical tasks regarding preparation of payment report concerning dividends, distribution of surplus, and interest on guaranty capital
    (iii) Clerical tasks regarding preparation of payment report concerning fees for real estate rentals, etc.
    (iv) Clerical tasks regarding preparation of payment report concerning compensation for real estate transfers, etc.
     
  4. Matters related to safety management measures
    Our company shall take safety management measures that are necessary and appropriate for the management of Specific Personal Information, etc., including preventing leakage, loss, and damage. Furthermore, our company shall carry out the necessary and appropriate supervision of employees and outsourcing contractors (including subcontractors, etc.) that handle Specific Personal Information, etc. Safety management measures regarding Specific Personal Information, etc. have been set forth in detail in our separate “Rules on the Handling of Specific Personal Information, etc.”
     
  5.  Disclosure of specific personal information
    When there is a request for the disclosure of the Specific Personal Information, etc. of an individual by that individual or his or her representative, our company shall accommodate such without delay, except in the following situations.
    (i) Doing so carries the risk of harming the life, limb, property, or other rights or interests of the individual in question or a third party.
    (ii) Doing so carries the risk of notably hindering the proper implementation of our company’s business operations.
    (iii) Doing so would violate laws or ordinances.
